Choose the Right Rehabilitation Center
When you or a loved one needs rehabilitation, it is important to choose the right facility for a successful recovery. Many facilities provide similar services, but the quality, experience and level of coordinated therapies can vary greatly.
Use the following checklist to help choose a rehabilitation center that is right for you. You can also download a printable version to take with you when visiting facilities.

| Is the facility accredited by The Joint Commission and the Commission on Accreditation of Rehabilitation Facilities? |

| Does the facility offer private, spacious rooms and amenities? |

| Does the facility offer spacious, well-equipped therapy gyms and advanced rehabilitation technology (such as Bioness®, Dynavision and VitalStim) to facilitate your fullest recovery? |

| Will your rehabilitation be directed by a board-certified physiatrist who is available to manage your medical needs 24 hours a day? |

| Does the facility regularly monitor outcomes? If so, are the outcomes available for your review? |

| Does the rehabilitation staff work as a team, conducting regular care reviews to ensure you are receiving the highest benefit from your rehabilitation? |

| Are there expert physical, speech and occupational therapists and certified rehabilitation nurses working with you on an individual treatment plan based on your specific needs? |

| Are rehabilitation services available seven days a week and required at least three hours a day? |
 |
|
| Do patients and their families meet regularly with physicians and the rehabilitation team to set goals and discuss progress? |

| Does the facility provide patient and family education and training? Will they refer you and your family to community services to help ensure a smooth transition home? |

| Does the facility coordinate your outpatient and/or home health services if you need further rehabilitation after you leave? |

| Is the facility equipped to provide immediate access to emergent medical care (such as imaging, dialysis, cardiology, neurology, etc.) without having to transfer out of the facility? |
