Review Your Records
If you would like to review your Atrium Medical Center (Atrium) medical record in person, arrangements can be made by appointment for you to do so in the Medical Records/Health Information Management Services Department in the presence of a Medical Records/Health Information Management Services Department employee.
You must bring a photo ID. You may not remove any part of your Atrium medical record from the folder or make any changes, but you can request a copy of your record for a reasonable fee.
Request a Correction
If a change needs to be made to your Atrium medical record, you can request a change by filling out an Amendment Request Form. Complete the form, specifying what information you want amended, then sign and return it to the Medical Records Department. You can also call the Medical Records Department at 513-420-5200 to request that the form be mailed to you. Either an approval or denial notice will be mailed to you after a decision has been made.

The Medical Records Department/Health Information Management Services Department is located on the second floor of the Compton Center. Park in Lot D and enter through the Professional Building entrance. Take the elevator to the second floor and turn left. Hours are 8 a.m. to 4:30 p.m., Monday-Friday. An appointment is preferred but not required.
